Incident Investigation / Root Cause Analysis and Report Writing Workshop

Program Overview:       

 Incidents are events that gave rise to or had the potential to lead to an accident, faults/malfunction of equipment, violations of business code of ethics and other interruptions to the operations of a business. “Root cause analysis (RCA) is one of many quality improvement approaches used to identify, understand and resolve any root causes of problems or incidents. It is basically a problem-solving technique. RCA is a relatively new methodology that is continually evolving.” Incident investigations and reporting are important tools for identifying and controlling the causes of incidents at the workplace the essence of this workshop.

Managers, Engineers involved in work process, process safety, mechanical, Operations and Maintenance Staff, senior operators, maintenance technicians, supervisors, Process reliability staff, Process quality control/assurance staff.

Learning objectives

At the end of the program, participants will be able to:
